What is Private Pay?
This service is ideal for managing all types of medical conditions, ensuring medication management, and providing advanced care with the utmost respect and professionalism.
Key features of Rivers of Hope’s Private Pay program include:
Personal Care Services
Assistance with Activities of Daily Living (ADLs) such as bathing, dressing, eating, ambulating, and toileting. The goal is to help individuals manage their daily needs with dignity and independence.
Nursing Oversight
Registered nurses provide regular health assessments, medication management, and coordination of medical services to ensure that each individual’s health needs are met.
Care Management
Care managers work with individuals, their families, and healthcare providers to develop and implement personalized care plans. They also provide ongoing support and adjustments to the care plan as needed.
Eligibility Criteria for Private Pay:
Age and Residency
Must be at least 22 years old and a resident of Massachusetts.
Residence in a Managed Building
GAFC services are provided to individuals residing in managed buildings that offer 24-hour access.
MassHealth Member
Must be eligible for and enrolled in MassHealth Standard or CommonHealth.
Exclusion of Room and Board
It is important to note that while GAFC covers personal care services, it does not pay for room and board.
Health and Disability Needs
Must have a clinical need with one or more ADLs as certified by a healthcare provider.
Eligibility Determination
The eligibility for GAFC is determined by MassHealth, not by Rivers of Hope.
