In Rivers of Hope, consisted of a multidisciplinary team the director, registered HHA, care manager and administrative manager provides the highest quality of services and advocacy for children and adults with intellectual, developmental, and physical disabilities. We offer monthly or frequently home visits. We also provide 24-hour coverage for emergencies.
Job Description
Duties:
Provide personal care and assistance to clients in their homes
Assist with activities of daily living, such as bathing, dressing, and grooming
Administer medication as directed by healthcare professionals
Monitor and record vital signs, such as blood pressure and temperature
Assist with medical documentation and maintaining accurate records
Help clients with special needs or conditions, such as mobility issues or cognitive impairments
Prepare meals according to dietary restrictions and assist with feeding if necessary
Perform light housekeeping duties, including cleaning and organizing client's living space
Provide companionship and emotional support to clients
Qualifications:
High school diploma or equivalent
Certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) preferred
Experience working in a home health or similar setting is a plus
Knowledge of medical terminology and basic medical procedures
Ability to follow instructions and work independently with minimal supervision
Excellent communication skills and the ability to interact with clients and their families in a compassionate and professional manner
