Nurse (Per Diem):

Nurse (Per Diem): Our selected candidates will be responsible and accountable for assessing, prescribing, delegating, coordinating, providing, and improving acute and specialized nursing care. Under the direction of a higher level RN, Nurse Manager, or Charge Nurse selected candidates will integrate performance improvement (PI) concepts, standards of nursing care, Rivers of Hope’s mission and applicable regulations, while providing care to clients from the community and their qualified settings.

We are looking LPN and Registered Nurse (RN) who will assess clients for services, determine clinical eligibility for programs and develop care plans. Our primary goal is to support individuals who receive services in the community, so they can continue living in their own home.

Position requirements:

  • RN licensed required.
  • Minimum 2 years’ experience working with the elderly and/or individuals with developmental disabilities.
  • Valid driver’s license and reliable transportation a MUST.
  • Travel to Plymouth, Bristol, Norfolk, and some of Essex, Middlesex counties
  • Acceptable results must be received for background checks including Credential Verification, Driver History, CORI, SORI and Fingerprinting
  • Knowledge of the theories and standards of Nursing Practice including the laws and regulations concerning patient rights and data collection methods for patient assessments.
  • Ability to empathize with patients and interact with individuals who are under physical and/or emotional stress.
  • Capacity to possess physical and mental endurance
  • Skilled in making decisions and acting quickly in stressful or emergency situations.
  • Proven ability to exercise discretion in handling confidential information (HIPPA).
  • Demonstrated knowledge of the types and characteristics of drugs used in providing nursing care.
  • CPR Certified
  • Ability to solve problems and relate to clients and hospital staff in a positive and professional manner.
  • Demonstrated commitment to patient care excellence and safety.
  • Skilled in utilizing critical thinking skills.
  • Talent for recognizing the cultural diversity and ensure sensitive and equitable treatment of all individuals including patients, staff, and visitors.

Preferred, not Required:

  •  Fluency in Cape Verdean Creole, Spanish, Haitian Creole, or other languages spoken by people in the communities we serve (where necessary)

 

Key responsibilities:

  • Provide exceptional nursing care including assessing, prescribing, delegating, coordinating, and improving acute and specialized nursing care to diverse and complex patients.
  • Develop a Plan of Care with patient, patient’s significant others, multidisciplinary health care team, and other resources.
  • Provide direction and participating in safe nursing care activities in collaboration with ancillary staff (Nursing Assistants, Licensed Practical Nurses, other personnel)
  • Review patient data to remain informed and knowledgeable about the dynamics of patient needs, strengths, abilities, and potentials.
  • Transcribe physician orders for medications/tests/interventions; administers and documents prescribed medications while monitoring and documenting responses.
  • Collaborate and cooperate with all disciplines in development of an inclusive plan of care for patients in accordance with nursing theory, departmental unit policies/procedures, CARF, VA, and federal and state laws.
  • Communicate information about patients to appropriate health care professionals (Physicians, Social Workers, Dietary and other clinical staff).
  • Attend required educational seminars to maintain licensure, and other workshops, in-services, and seminars to enhance own professional development.
  • Participate and contribute to departmental wide PI activities by assisting in data collection/analysis, identifying opportunities and improvement in care or services.
  • Demonstrate competence in assessing and providing care that is cognizant of, and sensitive to patients’ cultural and spiritual needs.
  • Completes and coordinates all applicable clinical assessments and clinical evaluations.
  • Following acceptance to program and ensures that all required forms and standard procedures are followed for the placement process.
  • Conduct on site home visits and complete LPN/RN progress note and other paperwork as required.
  • Ensure all documentation and forms are complete, accurate, and timely.
  • Excellent interpersonal, organizational, and communication skills.
  • Ability to independently manage a caseload and provide all encompassing care management.
  • Excellent documentation/writing skills.
  • Ability to think and work creatively as part of a team and/or independently.
  • Ability to manage challenging behaviors.
  • Perform other tasks as needed or assigned
  • Conducts orientation training, and on-going trainings to caregivers
  • Review medication changes, new diagnosis with caregivers and members
  • Plans and implements discharges and transition from program
  • Maintains office services by organizing office operations and procedures; controlling correspondence; designing filing systems
  • Follow all regulations and guidelines set by Medicare, state programs and ACO/MCO

 

Physical environment and demands: While performing duties for this position, the employee travels by using their own personal vehicle and is exposed to changing weather conditions. The employee must have the ability to safely travel to and from qualified work settings. In addition, the employee will be required to drive for meetings and/or visits provider homes or to facilities targeted for program marketing. Since AFC provider homes are private residences, the employee may be exposed to pets, family members, smoking, cooking, or other fragrances. The noise level in the work environment is moderate.
